What actually goes wrong
The bottleneck is almost never traffic. Clinics with a full enquiry inbox and an empty calendar are the normal case, because the enquiry arrives from someone comparing four clinics who has no way to tell them apart. Everything past that first message is a trust problem.
Second, the site is usually one language with a translated copy bolted on. A patient in Germany searching German terms does not find a page written for them; they find an English page that Google decided was close enough, and they leave.
